PROGRAM SPECIALIST IDD
Philadelphia/UPPER DARBY Service Provider is recruiting for a Program Specialist for our Adult Intellectual and Developmental Disabilities (IDD) Community Residential Homes in Delaware/Philadelphia County.

This can be a full-time or part-time position. This position requires the availability to work flexible hours when needed, which includes evenings and weekends, to meet individual and programmatic needs.

Role and Responsibilities

The Program Specialist is responsible for assessment, planning, and monitoring of services to ensure the health, safety, and welfare of individuals in our residential program in compliance with State of PA 6400 regulations and all other applicable State and Federal regulations.

The Program Specialist maintains overall supervisory responsibilities for all quality aspects of Residential Program operated by our company. This position also provides active supervision of the Residential Program Supervisors, Direct Support Staff, Team Leads and Direct Support Staff working in assigned homes who are engaged in assisting program participants to live and function as independently as possible within the community.

Essential Duties and Functions:

· Oversee quality of programmatic, health and facilitative services by ensuring that the needs of individuals served are met

· Advocate for consumer needs

· Meet with consumer's team to identify service needs and goals to ensure that such goals are accurately documented in the ISP

· Participate in ISP meetings and consumer reviews. Monitor annual ISP progress for all consumers

· Create, review and implement individual goals and data sheets based on assessment and ISP to monitor consumer progress

· Write instructional plans for the implementation of goals, objectives, and outcomes to implement consumer long and short-term goals and train all staff in the implementation of ISPs annually

· Provide staff training as it relates to service delivery requirements and expectations as outlines in each consumers ISP.

· Update staff in a timely manner every time service expectations change to ensure that all staff are properly trained and up to date on consumer plans.

· Attend and actively participate in all team meetings

· Organize, maintain and update consumer files in accordance with the DPW 6400 regulations

· Complete and review annual assessments and progress summaries for all consumers, all monthly and quarterly progress notes and lifetime medical histories

· Ensure that all consumers' medical, dental, psychiatric, and psychological services are scheduled in accordance with 6400 regulations and doctor recommendations

· Attend consumer medical and psychiatric appointments in the absence of Residential supervisory staff

· Ensure that all service delivery forms, individual goal sheets and data sheets and other forms as required by the regulations, employer and/or program are completed in a timely manner and accurately reflect the consumer's individual needs.

· Maintain open communication with individuals, family members, day programs, Behavior Consultants, SC's and all other outside entities

· Attend all monitoring visits of SC's and other outside monitors

· Coordinate and distribute all consumer financial matters such as SSI, food stamps, rep-payee-ship, burial accounts, taxes, SSA Monthly Wage Reports, Room and Board contracts, renewal of annual stipends etc. with Financial Administration

· Coordinate all communication for the consumers with the Social Security Administration, Medical Insurance Companies, and other outside entities

· Develop and implement a continuous quality improvement plan and data collection in conjunction with management team

· Develop and maintain a catalog of community resources/activities

· Conduct regular unannounced visits of all sites and share any necessary documentation

· Serve as incident management point person

· Assist with opening new homes and transitioning consumers into, within and out of the program

· Represent the program in a professional manner and model leadership and boundaries with staff and consumers at all times

· Complete all other miscellaneous duties as requested or assigned

Requirements

· Master's Degree in human services field plus on year experience working directly with individuals with Autism/MH/IDD, or

· Bachelor's Degree in human services field plus 2 years' experience working directly with individuals with Autism/MH/IDD

· Knowledge of HCSIS and EIM system

· Strong working knowledge of DPW 6400 regulations and licensing inspections

· Demonstrated good communication, organizational, leadership and time management skills

· Ability to engage clients and staff in effective, therapeutic and healthy interactions

· Demonstrated basic experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint

· Will be required to sit, stand and/or walk for extended periods of time and as necessary assist individuals in service with their daily needs

· Current, valid State of PA drivers license, 3 years driving experience, and acceptable driving record

· Current physical and TB Screening

· Current CPR and First Aid Certification

· Current Criminal Background Screenings

· Verifiable relevant professional references
