Addictions/Resource Emergency Department Manager, Greenfield
Job description
CSO is looking for an Addictions/Resource Emergency Department Manager to join out team.
This manager will provide recovery support navigation service for Emergency Department-involved individuals impacted by substance use. They will provide short-term care management and connections to long-term and comprehensive care coordination services, including the Baystate Franklin Addictions Service. Additionally, they will develop and maintain knowledge of community resources for sharing with individuals and supporting actions for recovery.
Responsibilities
- Explore treatment recovery options with clients, developing a set of goals and objectives in conjunction with the client based on identified needs, providing education and resources, and assisting clients in accessing treatment and community supports.
- Understand the evaluation, treatment, recovery support, family support and overdose prevention resources available in the community.
- Provide short-term care management and connections to long-term and comprehensive care coordination services.
- Develop personal goals and objectives related to substance use, treatment, recovery, and harm reduction.
- Assist clients with MassHealth enrollment, if needed.
- Assist with access to treatment and/or supports for achieving sustained recovery. Serve as liaison between person served, the Addictions Consult Service, Baystate staff, recovery community, including attending self-help meetings or other supportive services.
- Develop and maintain knowledge of community resources for sharing with individuals and supporting actions for recovery.
- Work with supervisor to integrate the recovery perspective into site programming, team roles, and individual care decisions.
- Assist to identify and obtain entitlements and provide advocacy which meets consumer's needs.
- Provide counseling and intensive support to consumers during psychiatric crises or periods of transition; assist consumer with identifying appropriate mental health and medical services and accessing services of choice.
- Appropriately support and facilitate transportation needs of clients, as needed.
- Help consumer develop and maintain a "natural support network" of family and friends, including self-help support groups and community-based recovery supports.
- Assist in referring client for appropriate housing support services.
Qualifications
Bachelor's Degree in a behavioral health discipline or related field; minimum one year demonstrated experience providing Recovery Support navigation services. Must possess good listening skills; ability to establish trusting relationships through compassion, empathy and insight, and ability to foster self-sufficiency using problem-solving and practical skills; familiarity with support groups or self-help education groups. Excellent oral and written communication skills and computer proficiency required including ability to work with email, electronic health record and word processing software. Must be able to work independently and follow through with projects as well as be an effective member of a team. Must be able to work flexible schedule which may include evening and weekend work, as required by client needs. Valid driver's license and reliable transportation required.
