Job description
The Texas Correctional Office on Offenders with Medical or Mental Impairments (TCOOMMI) Adult Probation Intensive Case Manager (ICM) is responsible for providing outstanding person-centered intensive case management services to adults diagnosed with severe and persistent mental health disorders who are currently on the specialized Mental Health Initiative probation caseload in Caldwell and/or Gonzales counties, Texas.
The position reports to the TCOOMMI Program Director & TCOOMMI Case Manager Supervisor. The position requires the development and maintenance of effective, professional working relationships with Caldwell & Gonzales Counties Adult Probation Officers and other community service providers. This is a Monday through Friday, non-exempt, primarily field-based position with flexible hours – no nightshift or on-call. This position involves the option to work remotely to complete only required administrative tasks and/or when telephone or televideo video services are deemed appropriate by supervisor. All remote working must be first approved by the supervisor or member of executive management team.
The TCOOMMI ICM regularly meets with individuals in their homes, community, probation/parole, court, and at Bluebonnet Trails’ clinics to provide case management and psychosocial rehabilitative services for a caseload of 20-25 individuals. Services are intended to stabilize mental health symptoms, reduce risk of recidivism, increase the individual’s awareness of and participation with community and natural supports, develop self-advocacy, and assist with their ability to participate in independent behavioral health services. 55% of hours worked are expected to include billable and non-billable service hours on a monthly basis to achieve 100% productivity percentages throughout the month. A high level of detailed electronic documentation is required and adherence to company and state guidelines is expected. The person hired in this position should be knowledgeable about the Texas Department of Criminal Justice, TCOOMMI Program Guidelines and Processes, PGP-01.02.
The person in this position must possess a passion and aptitude for working with individuals who have chronic and severe mental health needs and are involved in the criminal justice system. Knowledge of psychotherapeutic medications, intensive case management functions, substance use treatment, crisis services, community resources, and evidence-based practices in the mental health and criminal justice systems are required. This position necessitates innovative thinking, patience, resiliency, skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning and dual-diagnosis treatment. The person in this position will effectively work independently and on a team, prioritize needs during crisis situations, engage in regular meetings and training opportunities, balance self-care, be a delight to work with, and perform other duties as assigned with a positive attitude.

Salary: $20.44 – $26.19/hr

Education and experience in behavioral healthcare and/or the criminal justice system.
Must have a valid Texas Driver's License, meet Bluebonnet Trails Community Services driving requirements, and maintain approved driving status throughout employment.
Preferred Qualifications: Preferred Qualifications: Previous experience working in TCOOMMI, behavioral health, criminal justice, and/or case management; advanced degree in Human Services field; QMHP certification; Licensed Practitioner (LCDC, LCSW, LMFT, LPC); Bilingual (English/Spanish).
