ADVERSE INCIDENTS GUIDELINES AND PROTOCOLS CONSULTANT


Background:

Palladium has increased the number of projects focusing on improving delivery of health services. These projects are partnering with ministries of health at national and decentralized levels, in health facilities and in the community, to conduct assessments of clinical services, trainings in clinical service delivery (e.g., emergency obstetric and neonatal care, treatment of malaria in pregnancy, insertion/removal of IUDs), provide mentoring to clinicians, participate in supervision visits, and support quality assurance/improvement initiatives. In addition, projects are engaging communities to support increased use of services and holding health systems leaders and managers accountable for quality services. Engagement in such projects exposes Palladium to risks for confronting an adverse incident at a health facility, through delivery of community-based services, and/or through engagement with community residents. To manage these risks, Palladium has developed an adverse incidents policy with the aim of ensuring all adverse incidents are quickly identified, appropriately managed within their programs and locus of control, and that required reporting and investigation based on their severity are conducted. Palladium also intends to use experiences with such incidents to facilitate learning and making changes that will further improve safety for clients in Palladium-supported health facilities and community settings, and also minimize any legal or reputational risk that could result following an adverse event.
Palladium seeks a consultant to 1) develop guidelines to operationalize the policy, including reporting forms and instructions that can be adapted to match project context, and 2) develop orientation material for use by service delivery projects to help staff understand their roles, responsibilities, and reporting requirements.

Consultant Responsibilities:

This consultancy will have three interconnected products:

  • Drawing from the Palladium adverse incidents policy, develop guidelines that projects can use to operationalize the policy. These guidelines will provide instructions for how projects introduce the policy to staff, partners, and government counterparts at national and subnational levels.
  • An important aspect of policy operationalization is the specific reporting form and instructions that will allow staff to report an adverse incident. In coordination with the Palladium technical point of contact, develop a reporting template, ensuring that it meets the requirements for submission to the Palladium Risk Registry. Instructions should also be developed to guide staff on how to use the reporting form.
  • In order for staff to understand their responsibilities when they join Palladium and when this policy is initially introduced, orientation material is needed. The consultant will develop a presentation to serve as the orientation and it will include detailed speakers notes.

Time Frame:

All three products need to be finalized by August 1, 2023
