PROGRAM: Freedom House/ Louisville and SEKY STATUS: 1 FTE
POSITION: Freedom House Alumni Coordinator
REPORTS TO: Sr. Program Manager/ Program Manager
LOCATION: Manchester, KY

OVERVIEW:
Has the responsibility for building proactive relationships with all Freedom House clients at admission, during the clients’ treatment episode, and prior to discharge and collecting all outcome data following program exit. This person will be responsible for maintaining a relationship and connection to former clients, both those who have successfully completed treatment and those who have not. This person(s) will have the responsibility of supporting work for the project in both the Louisville and Manchester communities, using her own experiences and consultation with others on the program evaluation teams to establish and maintain access to community stakeholders, Freedom House clients, and outcome data. This person will have the responsibility of doing this work with a trauma-informed approach that is consistent with our philosophy of care.

REQUIREMENTS:
1) Graduate of Freedom House/Other VOA Recovery Program
2) Good verbal and written communication skills
3) Organized and dependable
4) Satisfactory criminal records check
5) Driver’s license and satisfactory driving record
ADDITIONAL REQUIREMENTS:
1) Ability to multi-task and prioritize projects, while working in a fast-paced environment.
2) Strong work ethic with a focus on process and results.
3) Ability to learn and apply agency and personnel policies and procedures,
4) Strong computer skills
5) Willingness to maintain some on-call duties/after-hour phone access.
6) Comfort working closely with others and constantly meeting new people
7) Ability and skillset of maintaining a trauma-informed approach that is consistent with the VOA Recovery philosophy of care

RESPONSIBILITIES:
1. Conducting outcome questionnaires at intake, discharge, 6 month post-discharge, and then recurring every 6 months up to 36 months for a maximum of 6 interviews/outcome measurements per client.
2. Distributing incentives to all applicable participants
3. Sending outcomes/loading outcome data in systems as needed
4. Conducting orientation and other groups as assigned with new Freedom House residents
5. Maintaining relationship and connection with all clients and former clients, to include being responsible for recognizing important milestones (birthdays, sobriety birthdays, graduation date, first day of school, etc) by sending cards and make supporting phone calls and texts, etc.
6. Identify current freedom house residents and/or recent graduates to participate in interviews
7. Assist with the organization of and participate in team meetings
8. Participate in analysis and dissemination activities
