The Behavioral Health High Acuity Care Navigator acts as behavioral health liaison among patients, clinicians, support staff, and outside agencies; and coordinates, documents, and tracks services and referrals for patients in crisis. Must exemplify the mission of SHHC and the Patient Centered Medical Home model of care on which it is built.

This position is hybrid.

Responsibilities:

• Acts as behavioral health liaison among patients, clinicians and outside agencies.
• Knows social service environment and refers patients as necessary.
• Coordinates services for patients in crisis.
• Acts as liaison between in patient psych hospitals and SHHC clinicians
• Educates patients and staff regarding referral process.
• Follows methodology to track patients, referrals, transitions of care, quality metrics, outcomes, and other behavioral health indicators in coordination with the BH Coordinator
• Obtains release of information to seek necessary records.
• Charts in EHR in accurate and timely fashion.
• Maintains confidentiality and is compliant with HIPAA.

Qualifications:

• COVID vaccine, Bivalent booster, and flu vaccine required.
• Bachelor's degree required, MSW, strongly preferred
• Commitment to connecting a diverse group of patients to community resources Knowledge of area social service networks, health insurance issues, and an interest in immigrant and refugee populations.
• Medical terminology, ICD-10 and CPT coding knowledge preferred.
• Excellent communication, computer, and organizational skills; and a strong mission orientation
• Collaborative team player
• Second language a plus
