Job Summary

Provides quality, cost-effective inpatient/outpatient and/or community care for clients and families in a target population across the continuum of care by assuring access, providing assessments and coordinating clinical summary treatment plan determining level of care. Assumes a caseload and performs behavioral health and/or substance use services encompassing community and ambulatory services and including Behavioral Health Services as scope of practice.

Job Responsibilities

• Provides direct client care using individual, couple, group and family therapy specific to each client’s needs as indicated on treatment plan.

• Preparesappropriate medical documentation to meet regulatory requirements of licensingand third party agencies.

• 
  Assessesand plans care by evaluating the psychiatric and psychosocial needs of theclient; organizes and prioritizes care and treatment and advocates appropriateresources for the client and family.
• 
  Providesdirect crisis intervention to clients and families in multiple sites in thesystem and in the community.
• 
  Providescontinuous education to client and family members and refers them toappropriate levels of care, to support groups and to other available resourcesin order to maximize care and facilitate recovery or treatment process.
• 
  Proficientin working effectively across programs for clients with both mental health andsubstance use disorders.
• 
  Negotiateslevel of care and length of stay with managed care companies; continuallyre-evaluates client’s needs and informs the third party payers of ongoingchanges in the client’s status.
• 
  Workscollaboratively as a team member along the continuum of care to provide anintegrated delivery of healthcare to all clients and families.
• 
  Participatesin administrative activities including, team meetings, clinical supervision ofstaff and staff education as designed by the Program Manager. Assumes responsibility for self-improvementby identifying, communicating and seeking resources to meet own learning needs.
  

Qualifications

• Master's Degree in Social Work, Counseling, or related field.
• Valid CT Licensed Masters of Social Work (LMSW) if Master's degree is in Social Work.
• Three years' experience providing direct services to psychiatric patients and/or substance abuse population.
• Previous experience in assessment and diagnosis and individual and group therapy.
• Demonstrated experience using a computer; basic keyboarding skills.
• Flexible with schedule per program needs, including evenings/weekends and travel between sites across organization.
