The purpose of this position:
Children’s Service Center is currently looking for an energetic and dependable person who works well independently and collectively as a treatment team member. If you are ambitious and confident in your ability to learn, the Blended Case Manager (BCM) (similar to mental health worker, social services worker, targeted, Intensive case management, resource coordinator) position is right for you. Join our team of employees as we continue to grow and experience the positivity of advocating for services as needed by adults with significant Mental Health diagnoses. You will be fully trained to work in a variety settings with a flexible schedule to closely monitor and advocate for services as needed. You will report to the Blended Case Manager Supervisor.
Responsibilities include, but are not limited to:
- Knowledge of the mental health and social service field
- Ability to use good judgement skills in assessing consumer needs and to be creative in seeking appropriate services
- Ability to organize time efficiently to meet client needs in a flexible, non- traditional time frame
- Become familiar with available community resources and keep up to date in their locations, eligibility requirements and programs

Qualifications:
Bachelor's Degree in Social Work, Sociology, Psychology or related social science field
OR
Associated Degree along with two years of relevant experience in a Social services field
Must pass Safe Crisis Management Training (physical and theory), upon hire and then annually as a condition of employment. Other requirements include:
- Driver’s License
- FBI Act 73
- Criminal history
- Act 33 clearances
