JOB SUMMARY

Do you want to join the St Leonard’s Hospice Team providing excellent care to patients from York and the surrounding area at the end of life?

  • *This is a great opportunity to consider a career move into palliative care as a supported induction and tailored development plan will enable you to gain the skills required. Previous experience of caring for patients at the end of life would be advantageous but not absolutely necessary as a supportive programme of learning will be facilitated as required.

You will be a dynamic, highly motivated individual who wants to make a difference both personally and professionally to the patients and families of those who require palliative and end of life care.
The role is ‘hands on’ with a need to deliver person centred, individualised care and social support to meet needs of patients and their families. You will be expected to travel to the patient’s address and stay with them for several hours at a time to allow their carers much needed respite and breaks from their caring role.
With the support of the wider Hospice at Home team this vital role will involve lone working to provide essential support to the carers of patients with Palliative Care needs. In allowing regular breaks for carers, at times to suit their needs, we are better able to support patients in meeting their end of life wishes.
You will be responsible for maintaining our Hospice ethos to ensure that high quality, personalised and holistic care is provided to the patients and their families.
Excellent communication and personal skills are required to develop relationships with those in your care and to ensure relevant and key information is shared with all staff involved in the care of patients. You should be able to respond appropriately to emotionally challenging situations.

We support all our care assistants to complete the nationally recognised Care Certificate – there is an expectation that you will complete this certificate within your first year of employment.
