Position Summary:
As the Care Coordinator at Malvern Community Health Services, Inc. ("MalvernCHS"), you will report to the Director of IBHS and be responsible for monitoring, coordinating, and reporting on administrative and clinical services and processes. Your role will require a multidisciplinary approach to ensure optimal outcomes in the areas of prevention, diagnosis, treatment, and transitions for persons served at MalvernCHS.

In this position, you will work independently and collaboratively with other programs, positions, and teams to improve processes that impact persons-served and their significant persons with their entry, orientation, continued care, program engagement, referral, and transition to and from Intensive Behavioral Health Services program at MalvernCHS. You will also monitor a person-served’s treatment episode, coordinate administrative, supervisory, and clinical support functions, and provide timely information to the treatment team and leadership staff that conform to MalvernCHS’ standards and applicable regulatory entities.
As the Care Coordinator, you may have occasional contact with children, youths, adults, their family members, their other significant persons, and representatives from professional and community settings. You will play a key role in creating an organizational culture that promotes high-performing, reputable, and evidence-based behavioral health practices.

Overall, this role requires strong organizational and communication skills, attention to detail, and a commitment to working collaboratively with other professionals to ensure the best possible outcomes for persons-served and their families.

Summary of Essential Position Functions:*
Coordinates clinical and administrative service delivery activities to persons-served, their family members, and other significant persons. This includes maintaining accurate caseloads, records, and effective communication with team members.
Submits correct and updated authorizations and monitors appropriate service utilization.
Coordinates referral, admission, eligibility, authorization cycles, billing activities, and discharge procedures.
Schedules appointments and manages their outcomes.
Conducts continuous review of health records and information, coordination documentations, and team communications.
Monitors a person-served’s overall well-being and provides support to address identified physical and behavioral health needs, such as their social determinants of health.
Provides support to families as an adjunct to the treatment services.
Coordinates data collection related to program and agency operations.
Collaborates across departments to achieve goals of quality assurance, compliance, integrity, and accreditation activities.
Coordinates meetings among team members, multiple agencies, systems, and disciplines. Participates in meetings, internally and externally, related to these essential position functions.
Performs all other related functions and responsibilities as assigned.
Qualifications:
Demonstrates desire and experience working with culturally diverse populations and individuals with mild to severe behavioral health challenges and multi-system involvements.
Strong organizational and time management skills; ability to prioritize functions and projects in a demanding environment; demonstrates flexibility regarding job duties and assignments; ability to effectively work independently and as part of a team to organize tasks, problem solve, and make decisions.
Excellent verbal and written communication skills demonstrated by the ability to communicate effectively and relate congenially with children and adults.
Proven understanding of standards and practices governing confidentiality and HIPAA regulations.
Thorough knowledge of and willingness to adhere to mandated reporting requirements as stated by law.
Working knowledge of generally accepted quality assurance and integrity standards and procedures.
Valid state driver’s license and ability to accept assignments as required.

Education and/or Experience:

High school diploma and twelve (12) semester credit hours in sociology, social welfare, psychology, gerontology, OR other social science.
OR
Bachelor’s degree from a college or university that is accredited by an agency recognized by the United States Department of Education or the CHEA in human services and recognized clinical discipline including social work, psychology, nursing, rehabilitation or activity therapies, preferred.
OR
Equivalent bachelor’s degree from a foreign college or university that has been evaluated by the AICE or the NACES. MalvernCHS will accept a general equivalency report from the listed evaluator agencies to verify a foreign degree or its equivalency.
AND
Two (2) years of experience in a health or human services setting, preferred or a combination of education, training and experience that provides the required knowledge, skills, and abilities to perform the essential position functions.

Credentials and Special/Technical Skills:*
Pennsylvania State Criminal History Report, Pennsylvania Child Abuse Clearance, FBI Criminal History Report, Act 31 Mandated Training, CPR/First Aid Certification.
Completion of mandatory individual and group supervision, training, and staff meetings as determined by the Director of IBHS and regulatory entities.
Strong customer and personal service skills.
Proficient in the use of Microsoft Office 365 (Outlook, Word, Excel, Teams, etc.), Google Chrome, and Adobe Acrobat Reader DC.
Must become proficient in the use of MalvernCHS’ electronic health records, human capital management, learning management, and telecommunication systems within one (1) month of hire.
Knowledge of behavioral health challenges and systems, managed care environment and trends, and community resources.
Must be able to intervene and control agitated persons through non-physical interventions. Hazards include risk of exposure to disease and risk of physical injury associated with served persons who have the potential to act out.
