Mission:

The Fresno-Madera Area Agency on Aging care management programs provide services to eligible participants age 60 and older that enable them to remain in their homes by preventing or delaying placement in a nursing facility.

Responsibilities:

All new participants must receive an initial face-to-face home visit psychosocial assessment by the care manager. This assessment also includes a cognitive screening, depression screening, assessment of the participants ADL's and IADL's, as well as an environmental assessment.

The care manager is responsible for monitoring the health, safety and welfare of the participant. The care manager is responsible for discussing with the participant or their representative, the participant's health status, care being provided, and any other significant changes. Each problem statement must be addressed monthly as it is the focal point of care plan monitoring.

The care manager is responsible for linking older adults to: Healthcare Resources; Adaptive Equipment; Nutrition Programs; Counseling Services; Minor Home Repairs; Recreation Activities; Transportation; Housing Assistance; and Caregiver Support Services.

Education Requirements:

BA/BS in social work, psychology, rehabilitation counseling, gerontology, sociology, or related field and a minimum of one year of experience in a health or social services specialty.

Additional Requirements:

Valid California driver's license, access to a car, and a clean DMV record.

Pay: $44,713.00 - $57,066.00 per year
