BayCare Health System is currently in search of our newest Case Management Assistant who is passionate about providing outstanding customer service to our community.

The Case Management Assistant - Hospital, under the direction of Care Coordination department:

  • Assist with completing tasks necessary to facilitate patient and family access to agencies, transportation services within and beyond acute care.
  • Facilitate inboard calls and assist patients' needs to support care coordination in the ambulatory setting.
  • Assist with distributing regulatory documents to identified patients
  • Educate patients about the appeal process related to their discharge
  • Perform multiple activities that support a seamless transition of patients from the inpatient hospital setting
  • Knowledge of hospital processes and community service.
  • Frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives.
