Summary

This position is responsible for providing care management services to Michigan Medicine/St. Jo patients who transition from inpatient care to the sub-acute rehab facility and ultimately to the community setting. The Case Manager interacts daily with MM Sub-Acute provider team, the SNF social work and clinical team and the patient/family/other supports with the focus of achieving desired quality outcomes. Serves as an internal consultant to SAR staff regarding care management, clinical and community resources. Daily onsite presence at the facility to accomplish position responsibilities. Reports to Program Manager.

Responsibilities*

  • Completes patient screening/assessment on all MM/St. Jo patients admitted to the SAR, identifying potential needs for safe and timely transition to home.
  • Interacts daily with MM Sub-Acute provider team, identifying planned discharge date based on patient medical condition and care goals.
  • Attends patient care conferences at facility, supporting plan for discharge.
  • Communicates with members of the multi-disciplinary team to discuss the patient’s course of progress and needs as well as clarify diagnosis, prognosis, and therapies, and suggest adjustments to the plan of care as appropriate.
  • Works cooperatively with SAR staff, overseeing patient progress in obtaining a safe transition to the community setting according to the planned medical discharge.
  • Identifies high-risk patients and assess treatment options and opportunities to improve the quality and efficacy of care.
  • Serves as a post-acute care program and community resource to the SAR staff and patient/family.
  • Pursues resource development to assist patients with identified barriers, working with facility social work staff.
  • Function as the patient’s ongoing liaison ensuring adequate communication between the patient and care providers.
  • Provides patient/family education as needed regarding community resources, entitlement programs and PAC services.
  • Maintains all required documentation in the appropriate electronic systems on a timely basis.
  • Manages assigned workload within established performance standards.

  • Actively engages the patient and family, as applicable, by exhibiting respect and dignity, sharing information in a timely and unbiased manner, and welcoming and encouraging participation to achieve shared decision making.
  • Collaborates with other members of the team, including the patient and family, to achieve the ideal patient care experience.


Other Related Duties:

  • Practices within the scope of license, certification, or registration.
  • Complies with all applicable continuing education requirements.
  • Complies with all University of Michigan and departmental standards and expectations.
  • Supports the department’s commitment to continuous improvement activities and principles through positive and respectful contribution in all problem-solving and change processes at Home Care Services.
  • Performs other duties as assigned.

Required Qualifications*

  • Bachelor’s degree in Nursing, Social Work, Psychology or other related field.
  • Current licensure or registration.
  • Two or more years of progressively responsible experience in care management.
  • Knowledge of community resources, governmental -funded programs, and health plans.
  • Ability to travel from point A to point B throughout the workday as assigned.
Physical Requirements:

The employee will be able to perform the assigned duties and responsibilities with or without reasonable accommodations: ability to work in a sedentary position for long periods of time, ability to communicate/ exchange information, fine motor coordination/dexterity necessary for keyboarding or typing, and may require reaching, bending, twisting, stooping, moving about, lifting of up to 10 pounds, etc., with or without the use of assistive/adaptive devices.

Desired Qualifications*

  • Strong interpersonal communication skills including problem solving and decision making.
  • Strong organization, written and verbal communication skills.
  • Strong dedication to customer service and ability to work within a team-focused, participative management framework is required.
  • Demonstrated computer competency, including EMR.

Background Screening

Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third-party administrator to conduct background screenings. Background screenings are performed in compliance with the Fair Credit Report Act. Pre-employment drug testing applies to all selected candidates, including new or additional faculty and staff appointments, as well as transfers from other U-M campuses.

In addition to the screenings indicated above under Michigan law, a criminal history check including fingerprinting is required as a condition of transfer or employment for this position.
