Belmont Behavioral Hospital currently has an opening for a Full time, Clinical Case Manager with a focus on serving our Adult Partial Hospitalization Programs and Intensive Outpatient Programs. As a clinical case manager in our PHP, this position will include supporting patients while in programming and coordinating care with external supports such as school and family, while focusing on aftercare planning and discharge resources to ensure the smooth transition of the patients back into their communities. This positon provides the ability to gain experience and skill across multiple levels of outpatient care while working as a member of a multi-disciplinary team.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

· Master’s degree in Social Work, Counseling, or related human service field required.

· Two or more years of experience working in a behavioral health setting is required.

· Previous experience in case management preferred.

LICENSES/DESIGNATIONS/CERTIFICATIONS:

· Licensure or certification for counseling, as may be required by the state where the facility operates.

· CPR and de-escalation/restraint certification required (training available upon hire and offered by facility).
