Case Manager – Family Connection Network
Hourly: $20.91 - $23.32
The Family Connection Network Kinship Care Case Manager will provide face to face home visits to assess the strengths and needs of relative/kinship families and develop goals for support of the home based on the needs of the kinship caregiver and youth. They will also assist with arranging and securing resources as needed, educate and encourage families, provide client-centered, culturally competent, and effective services to families caring for relatives served by the agency. The Case Manager will conduct after hours events as required but no on call crisis duties.
This hybrid role offers a flexible work schedule with both in office and at-home work available. Hour expectations are between thirty (30) to forty (40) hours per week, and this role is benefits eligible.
The Family Connection Network Case Manager will act as a member of a larger treatment team by:
Coordinating and mobilizing community resources for the family
Conducting face to face home visits with the family in order to assess needs
Developing rapport and using a strength-based approach with the families and children
Conducting interventions and teaching strategies to assist the relative/kinship caregiver (s) with managing behavioral and emotional challenges
Assist caregivers with concrete supports to reduce financial barriers in caring for kin
Position Requirements:
A Bachelor’s Degree in social work, psychology, sociology, or a directly related human service field from accredited college.
If BSW (Bachelor of Social Worker) or MSW (Master of Social Worker), licensure is required.
Preferred Requirements:
Minimum of two years previous experience in a social services agency
