Case Manager – Mecum House

Lighthouse Youth & Family Services has just the role for you. We are in need of a
Youth Shelter Case Manager for our youth from ages 10-17. This is a fulltime (40hrs) role working eight hours a day, five days a week.
Starting Hourly Rate: $19.04
Essential Job Functions:
- Ensures the awareness of both Youth Crisis Center and Lighthouse Sheakley Center for Youth as a resource to run-away and homeless young people, ages 10 up to their 25th birthday, in targeted areas of the community. This includes school presentations/ visits to schools, community centers, libraries, other locations where youth in a housing crisis are known to frequent.
- Provide the day-to-day management of the Resource Center, a drop-in service for homeless youth including answering phones, sorting mail, and front door greeting.
- Available to assist in facilitating life skills and psycho-educational groups to residents to enhance their knowledge of community resources and further development of self-sufficiency skills
- Provide initial intake and screening of all new residents to assess for motivation and barriers to housing and the development of an Individualized case plan.
- Assume primary responsibility for developing, writing, implementing, transition and revising treatment plans while utilizing team problem solving technique
Qualifications:
- Bachelor’s Degree in Social Work or related field required, BSW preferred.
- Minimum of 1 year previous experience working with homeless, at risk youth or working in a social service agency is preferred.
- Will prefer candidates who have experience working with harm reduction principles, particularly with regard to sexual risk, substance abuse, mental illness and LGBTQ population.
- Must have good verbal and written communication skills; organizational skills.
- Proficient in Microsoft Word and Excel
- Must have dependable transportation, a valid driver’s license and up-to-date auto insurance
