Overview:
The Case Manager is responsible organizing and delivering high quality case management services to children accessing care in the Physical Therapy department and their families. Services include case management and service coordination, both aimed at improving the level of child and family functioning and access to care.
Responsibilities:
  • Organize and deliver comprehensive, high quality case management services in accordance with program mission and design.
  • Provide needs-based assessment of case management client needs.
  • Collaborate with families, community participants, and other professionals to develop and implement case management plans that address the needs identified by clients and the department.
  • Deliver home and community based case management interventions as determined by the team. Provide referral services and recommend various resources within the community depending on family needs.
  • Provide service coordination and integration with other community agencies on behalf of clients by maintaining relationships with service providers, agencies, and other relevant organizations.
  • Participate in the process of identifying potential community resources.
  • Attend team meetings, patient rounds, and other case planning meetings on behalf of clients.
  • Maintain charts in accordance with the department and program requirements.
  • Maintain regular and consistent contacts with clients around identified case management goals.
Qualifications:
Must possess and maintain cardiopulmonary resuscitation (CPR) certification.


EDUCATION:

Bachelor’s degree in a behavioral health science or related field is required.

EXPERIENCE:

Previous experience working with families and children affected by abuse and neglect is strongly preferred.
