Seeking a full-time RN Case Manager who is passionate about providing quality care to patients and families in various residential settings including homes, assisted living facilities,and skilled nursing facilities in the Kitsap County, WA area. Case Manager (RN or LPN) needed!
Location: Bremerton, WA
Pay range: $85,000-$100,000/yr,
depending on experience

Qualifications:
- Current RN license to practice in the State of WA.
- Current WA driver’s license
- Reliable transportation
- Maintain current CPR certification
- Excellent communication skills

RN Case Manager responsibilities include:
- Coordinating all aspects of the interdisciplinary care and services provided to hospice patients andtheir families.
- Provides education to families and caregivers
- Develops, implements and reviews healthcare plans
- Advocating for patients and their families
- Documentation of care
- Nursing assessments
- Symptom management
- Promoting comfort and quality of life.
