Trinity Integrated Care is seeking a dedicated and experienced Case Manager Supervisor tooversee our team of case managers at our behavioral health facility. The Case Manager
Supervisor will be responsible for managing, mentoring, and providing guidance to the case
managers to ensure high-quality member care.
Responsibilities:
- Supervise a team of case managers, providing guidance, support, and feedback
- Navigate and utilize electronic health record (EHR) systems to track member progress
- Develop and implement care plans, in conjunction with the case managers and clinical
- Conduct regular meetings with case managers to discuss member care and provide
- Maintain accurate and up-to-date member records and documentation
- Run therapeutic groups and workshops to support member recovery and mental health
- Bachelor's or Master's degree in social work, psychology, nursing, or a related field
- High level of computer literacy and ability to navigate and utilize electronic health
- At least 3-5 years of experience in case management, preferably in a behavioral health
- Experience in supervision and management of staff
- Ability to write comprehensive assessments and reports
- Experience running therapeutic groups and workshops
- Ability to work collaboratively with other healthcare providers and departments
- Knowledge of regulatory requirements and policies related to member care
- Ability to multitask and prioritize work effectively
