JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Responsible for the delivery of clinical services to children/youth under age 21 and their families. Such services include assessment, individual, group, couples and family therapy and case management.
QUALIFICATIONS:
- Master’s Degree from an accredited college/university in a Human Services profession.
- LCSW, LPC or LMFT, or licensure eligible preferred.
- One year post-Master’s experience with children/youth/families.
- Excellent interpersonal skills.
- COVID-19 Vaccination or willingness to receive vaccine required.
SPECIFIC DUTIES:
- Provide direct clinical treatment including individual, couple, family, and group therapy using approved treatment modalities and interventions
- Provide case management services as needed for assigned children/families
- Maintain liaison contacts with schools and other agencies involved with assigned children/families
- Be aware of assigned youth’s financial liabilities, coordinate any financial changes with the Billing Department, and assist families in securing third party resources when indicated.
- Complete all required documentation including treatment plans, assessments, annual reviews, progress notes and discharge summaries in a comprehensive, timely and legible manner and in accordance with PATH policies and procedures
- Be flexible in scheduling appointments to meet children and families needs
- Contribute to any quality improvement/ outcome measurement tracking and data gathering as assigned
- Meet productivity standards for your position
- Work 2 – 3 evenings per week.
