Schedules Available:

  • Hybrid role- 8:30 am-5:00 pm
  • Remote work 2-3 days per-week

Essential Functions:

  • Complete ASAM Forms (admission, continued stay, transfers and Discharge) for each level of care. Documenting Criteria.
  • Develop a treatment plan, goals, objectives and interventions.
  • Participate effectively in multidisciplinary treatment plan reviews, demonstrating adequate preparation and understanding of patient issues, progress, areas of resistance, positive and negative strategies, etc.
  • Utilize experiential techniques effectively.
  • Assist patients gain insight into problems.
  • Maintain regular contact and open communication with families.
  • Demonstrate good strategic and problem-solving skills and utilizing morning meeting and group supervision to staff a case, if needed.
  • Be effective in finding needed resources and working in tandem with Case Management.
  • Ensure that documentation is timely, consistent, clear and articulate and never copy/pasted from another note or another chart.
  • Prepare patient to maintain recovery post-discharge. a continuing care plan that meets patient needs for ongoing support following discharge.
  • Complete Biopsychosocial, CSSRS and Eating Disorder Assessments
  • Complete Safety Contracts
  • Inform staff of pertinent client issues from assessments
  • Review/ sign documentation of unlicensed staff.
  • Oversee the QA committee-assign charts for Audit and maintain the QA meeting binder
  • Complete chart audits and inform staff of deficiencies
  • Monitor major spikes on Trac9 assessments and inform staff and provide guidance on follow up
  • Assess for involuntary hospitalization
  • Inform nursing of clients that have psychiatric diagnoses with no medication accompaniment (or vice versa)
  • Train/ mentor staff and interns on documentation/ assessments

Qualifications:

  • Master's Degree in Social Work (or a related Human Services field).
  • LCSW, LMFT, LMHC or LPC .
  • UR experience is preferred but is not required.
  • Working knowledge of JCAHO standards, levels of care and crisis intervention is strongly preferred.
