CLINICAL DIETITIAN-DIVERSIFIED SERVICES: Positions provide nutritional assessments, counseling and follow up. Oversees diabetes education program. Might provide any of the following based upon program, physician and department needs: develop nutrition awareness programs, provides instruction, treatment, education materials, consultations, diet analysis, cooking demonstrations. **In Medical Home Practices (PCMH/CPCI), this role may also work with patients of multiple chronic disease states including Diabetes, Obesity, Heart Disease, Hypertension, and well states to achieve optimal outcomes post-surgery or per consult request per physician.MINIMUM REQUIREMENTS:
EDUCATION: Bachelor's Degree Healthcare, Dietetics, or Nutritional Science BLS/CPR (Basic Life Support for Healthcare Providers) CDR Registration & Ohio Licensure. If needed, dietetic limited permit is accepted upon hire; required to pass the RD exam within 4 months of hire. Applicants not on a limited permit are required to have both licensure and AND registration.
EXPERIENCE: 1-2 years in a related field
SPECIALIZED KNOWLEDGE: Excellent written, verbal and interpersonal communication skills, computer application skills.
