Who We NeedImmediately Hiring a Community Behavioral Health Specialist, Monday through Friday 8:00am - 5:00pm.
Provide Supported Employment and Housing Services and Housing support to members with a diagnosed Mental Health challenge (SMI) seeking to obtain and/or maintain competitive employment in the community. Follow the 7 Supported Employment Core Principals of SAMHSA (zero exclusion, member preference, rapid job search, competitive employment, integrated services, long-term supports and benefits planning). Adhere to the philosophy of normalization and community integration as appropriate. Attend meetings on assigned caseload; communicate with clinical team any concerns and or accomplishments, presenting information in a professional manner. Be aware of individuals’ physical restrictions and medical concerns. Be aware of normal safety routines and procedures as they apply services. Ensure that all equipment utilized is properly maintained.
Requires any combination of training, education, and experience listed below:
- Require HS Diploma/GED plus four (4) years of full-time direct care experience in behavioral health.
- Bachelor’s degree highly preferred.
- Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire.
- Must have reliable transportation, valid AZ Driver’s License with state minimum insurance coverage and ability to meet and maintain Copa Health’s acceptable driving requirements.
JOB SUMMARY (INCLUDING, BUT NOT LIMITED TO)
- Functions as an integral part of an interdisciplinary team to develop individual housing plans.
- Conducts intakes and assessments, determines eligibility, and develops individualized case plans.
- Collaboratively work with housing specialists to find permanent housing options for program participants
- Conduct job development and job search activities directed toward positions that are individualized to the interests and uniqueness of the members on his/her caseload following the principles and procedures of Copa Health’s Supported Employment Program.
- Links individuals/families with community-based housing, utilizing the Housing First/Permanent Supportive Housing, EBP.
- Educate and maintain knowledge of community resources that serve individuals with disabilities. Educates and utilizes community resources.
- Manages financial assistance funds (security deposits, rent etc.).
- Determines and recommends to supervisor the level of financial assistance for eligible participants.
- Prepare requests for financial assistance and submit all required documentation to supervisor for approval.
