Teaching: Provide meaningful instruction to the individuals to assist with skills related to living in the community. These skills may include assistance with activities of daily living, personal care, health care, community inclusion, relationship building and development of social and leisure skills, accessing community services and supports, developing independent community travel skills. This instruction may be provided through formal habilitation goals as well an informal interactions and teachable moments.

Counseling and Problem Solving: Meets with individual as necessary to discuss behavioral concerns/maladaptive behaviors, problems identified by the individuals and/or sensitive subjects requiring attention. Encourages individual to identify potential outcomes and problems assistance with problem solving training and support as needed. Works with individual to appropriately communicate with outside entities such as Care Coordinator vendors, doctors, employment specialist, landlord, etc.

Community Activities: Assists the individual in coordinating a variety of leisure and community activities (such as adult education enrollment). Provides necessary level of training and support for individual to participate in their community. Transportation to and from activities will be reimbursed following federal guidelines.

Health and Safety: Is trained on basic emergency procedures such as first aid and contacting 911 as well as identifying and reporting abuse. Assist individuals with obtaining appropriate medical care and identifying and reporting changes in medical conditions.

Reporting/Documentation: Responsible for documenting all services delivered at the time of delivery in an objective and professional manner. Maintains a service delivery note in the individual’s progress on formal goals. Maintain progress notes when making collateral contacts/notifications. Works with immediate supervisor, individual and family member (as appropriate) to provide a continuous flow of pertinent information regarding the individuals progress and status changes.

Nassau: Long Beach, Levittown, Merrick, Freeport, Wantagh, Roosevelt, Farmingdale, Elmont , Baldwin, Oceanside

Suffolk: Seldon, Central Islip Bayshore, Southhold, Coram

Shift: Flexible – Many hours available; evenings and weekends. Starting at 3:00PM on week days is a plus!

Typical work hours: 3/4PM - 8/9pm or 10am-2pm; CURRENLTY HIRING FOR WEEKEND SHIFTS

Additional Needed Shifts:

Great Neck Monday thru Friday 9am-4pm
