Job Summary:
Provides support for patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports clinical and public health providers through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of patients and their family. CHW provides social support and peer health coaching, advocates for individuals and community health needs, and provides services such as basic first aid, blood pressure screenings, and simple wound care.

Knowledge, Skills & Abilities
Exemplar customer service; clear communicator including attentive listener; compassionate about the wellbeing of others; community-oriented; passionate about personal, familial, and community health; punctual; written and oral fluency in English; basic computer skills; ability and willingness to provide emotional support, encouragement, and motivation.
RESPONSIBILITIES
- Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement.
- Deliver the National Diabetes Prevention Program and other evidence-based lifestyle change programs.
- Deliver educational programs designed to address patients’ social needs such as health literacy, digital literacy, and financial literacy programs.
- Record patient care management in Epic no later than 24 hours after contact.
- Provide ongoing follow-up, basic motivation, peer health coaching, and goal setting with patients/families.
- Follow-up with patients via phone call, videoconferencing, home visits, and visits to other settings where patients can be found.
- Guide patients in scheduling and attending appointments.
- Provide referrals for services to community agencies as appropriate.
- Guide patients in connecting with transportation resources and provide appointment reminders.
- Exhibit excellent working relations with patients, visitors, staff, and providers.
- Work closely with clinical and public health providers to help ensure that patients have comprehensive and coordinate care plans.
- Knowledgeable about community resources appropriate to the needs of patients and families.
- Act as a patient advocate and liaison between patients, families, and community service agencies.
- Attend regular staff meetings, trainings, and other meetings as requested.
- Manage assigned caseloads of patients.
