Community Support Specialist Essential Functions:
- Conduct on-site visits with each client and caregiver at the qualified setting
- Obtains a social history and conducting a psycho-social evaluation
- Conducts the initial and ongoing evaluations of the AFC and/or GAFC setting
- Maintain and update client charts, medical records and submit all relevant information to authorized organizations
- Work collaboratively with the interdisciplinary team to create, monitor, and implement a client-centered care plan
- Assist participants with finding community, financial and legal services
- Partake in both care plan creations and reviews, along with discharge plans
- Assess and address motivational, psychosocial, and environmental concerns
- Conducts weekly calls to AFC Caregivers to confirm compliance with program
- Performs any other additional duties that may be required by management at any time
Community Support Specialist Requirements:
- Travel is required
- Valid MA Drivers License and reliable transportation required
- 1 Year of experience working with the geriatric or behavioral health population
- population or in a medical setting (ex. Hospital, Nursing Home, Home Health, Adult Day Health, or Community Health Center)
- Fluency in both English & Spanish highly perferred
- Strong communication, writing, organizational, and time management skills
- Case management, social services, and/or community health worker experience preferred
- Solution-oriented with the ability to multitask
- Proficiency with the Microsoft Office Suite
