Direct Support Professional/Mentor: Provides transportation to and from activities; Assists with the implementation of individual plans for individuals and behavioral plan goals; Assists individuals in the pursuit of recreational, social and community activities based on their preferences and individual plans; Assists and encourages individuals in the establishment of social networks; Promotes participation and involvement in community outings; Works on client’s goals & documents the goals, interventions, & outcome onto computer program within 24 hours of working with the client; Ensures each individuals’ rights are protected; Treats individuals with dignity and respect. Works closely with client and their families to support the client's needs. For this client we are looking for a DSP to work on Sundays from 9am to 4pm in Morehead City. CNA’s are encouraged to apply.
