Primary Responsibilities

  • Coordinates referrals and admissions to the facility through contact with the hospitals, other health care facilities and referring agencies
  • Perform comprehensive pre-admission clinical and fiscal assessments, both onsite and telephonic for the purpose of determining an admission to Mayflower Skilled Nursing Center
  • Completes internal admissions process
  • Tallies 72-hour resident satisfaction survey results and meets with departments to discuss and implement a plan of action to correct any problems addressed
  • Maintains and analyzes referral, admissions, discharge and satisfaction survey reports at Quarterly QA meeting
  • Provides informational tours of the facility
  • Develops and implements an annual marketing plan for the facility

Education/Experience/Licensure/Certification

  • Must have graduated from a state school of nursing must hold and continue to maintain a current license as a Registered Nurse in the State of Massachusetts.
  • Must possess at least three years of clinical experience in an acute care setting.
  • Must possess at least one-year experience in the following areas:
  • Skilled Nursing Facility
  • Discharge planning, Admissions, Case Management, Managed Care or Medicare
  • Excellent organization and time management skills
  • Must possess a strong communication skill and demonstrate the ability to work collaboratively in a team environment
  • Must possess the ability to communicate and facilitate in a professional manner between patients, agencies and personnel
  • Must possess the ability to interpret care contracts and guidelines
  • Driver requirements: Valid driver’s license with good driving record. Driving required as needed

Maplewood Senior Living conducts pre-employment screening including background check, drug screening, and reference checks.

Due to Department of Public Health requirements, all candidates will be required to show valid proof of full vaccination for COVID-19 (1st vaccine, 2nd vaccine, and booster) before commencing work, unless they receive an approved exemption from Maplewood. Those who do not obtain full vaccination and booster within that time frame, without an approved exemption, will be terminated.
