Regardless of role, we recognize that every member of our team is vital! The Discharge Recovery Coordinator provides outpatient case management to clinic patients. Their primary responsibility is to help co-create a recovery plan for each patient and to support the needs of patients as they prepare to graduate from the intensive expert care of a specialized University Clinical program to ongoing support within the community. The Discharge Recovery Coordinator is a member of the multidisciplinary treatment team and works closely with- and serves as a connector for- the patient, family, RN care coordinator, prescriber, therapist, primary care provider, and various community agencies. This role is centered around resource connection and community collaboration to support patients’ ongoing recovery and community integration needs.
Discharge Recovery Coordinator Overview
Key responsibilities include but are not limited to:
  • Closely follow and support patients as they progress through their treatment plan, in collaboration with their treatment team, to include adhering to their appointments and developing their graduation and discharge plans
  • Coordinates care across the health care continuum within designated sub-specialty population(s)
  • Serves as point of contact by facilitating and supervising transitions in care within and between care settings, providers, and services
  • Resource navigation for meeting the patient’s identified needs following discharge from the clinic
  • Complete required documentation related to closing patient care in the clinic and transferring their care to community providers
  • Establishes a collaborative relationship with physicians, medical directors, nurses and other unit staff, community partners, and payers
Minimum Qualifications:
  • Bachelor level degree in human services or similar. i.e. Social Work, Psychology or Human Services
  • Two-years experience in mental health and/or case management experience
  • Medical terminology; understanding of confidentiality and protected health information
  • Demonstrated ability to work collaboratively in the context of competing demands
