Disease Surveillance Coordinator
Three Rivers Public Health Department is seeking a dynamic Disease Surveillance Coordinator to join our amazing team. This position is responsible for program planning, implementation, evaluation and assuring compliance with state guidelines and policies for communicable disease control. We are looking for a self-driven, detail-oriented individual who wants to make a difference in our communities.
Essential Functions:
- Responsible for the delivery of appropriate disease control activities as defined by standard of care guidelines and applicable state and federal rules, regulations, and policies.
- Responsible for the development and management of a communicable disease reporting process that meets the needs of the department and is in compliance with all state and federal requirements.
- Monitor and track communicable disease cases in the area.
- Work with the health care community to facilitate reporting and care of individuals with a communicable disease.
- Receive and review notifications of all lab-confirmed communicable diseases and conduct public health investigations through utilization of the National Electronic Disease Surveillance System (NEDSS).
- Partner with local hospitals, health care providers, nursing homes, and schools to receive prompt notification of reportable diseases to begin the disease investigation process in a timely manner as well as to disseminate helpful information to prevent their further spread.
- Collect real time information on symptoms that may indicate local disease outbreaks through syndromic surveillance in area schools, nursing homes, hospitals, and physician’s offices.
- Interview, educate, and counsel clients diagnosed with or exposed to communicable diseases.
- Obtain lab reports from hospitals and physicians offices for the conducting of communicable disease investigations.
- Development of a working relationship with the state, federal and local agencies.
- Establish an on-going relationship with local medical providers to include, but not limited to, doctors, nurses, pharmacists, laboratory personnel, and hospital, school, county government, and day care staff.
- Advise medical providers regarding current laws, diagnostic procedures, and proper medications.
- Ensure that clinical documentation regarding clients is kept secure, confidential and maintained consistent with Health Department policies and procedures.
- Coordinate staff in assisting with other related programs, such as conducting employee health risk appraisals, health fairs, and general health education issues.
- Provide necessary planning, activity reports, work plans, budget submissions, and/or other required documentation for grant funded programs.
- Analyze statistical data in order to maintain and improve the status of community health and wellness.
- Attend trainings to obtain and maintain certifications required.
- Review documentation on charts and disease report forms.
- Elicit required statistical and demographic data from patients.
- Prepare reports of investigations.
- Conduct West Nile Virus Surveillance through monitoring of communicable disease cases along with collection and testing of birds.
- Utilize Geographic Information System (GIS) Mapping to view, understand, question, interpret, and visualize data in many ways that help to reveal relationships, patterns, and trends in the form of maps, globes, reports, and charts.
- Utilize GIS to answer questions and solve problems by looking at data in a way that is quickly understood and easily shared.
- Professionally represent the organization.
- Perform other duties as assigned.
Employment Qualifications:
- Bachelor’s degree in public health, biological sciences, natural sciences, epidemiology, community health, business or public administration, or other related field. Master’s Degree preferred.
- Two years of experience in public health, preferably in a health promotion/education capacity.
- Bilingual candidates preferred, but not required.
