Blaze Health (Minneapolis, MN) is seeking an experienced and dynamic primary care physician (FM or IM) to join our Primary Care Team, at our North Memorial Health Clinic - Brooklyn Park.

Position Purpose

The Clinic Physician provides direct customer care in the clinic office setting, acting as a member of the treatment team, ensuring the delivery of quality care. The physician will negotiate the visit agenda with the customer in order to best meet customer expectations.

Position Qualifications

• Completion of accredited ABMS approved residency
• Graduate of an accredited Medical or Osteopathic School
• Ability to obtain a Minnesota Medical Doctor or Doctor of Osteopathy license
• Valid DEA Registration
