Job description
To provide specialist family /carer and social work support within Palliative Care /LUHFT. The role is predominantly based within a hospice, the family support worker will take referrals from the Integrated Mersey Palliative Care Team (IMPaCT) in accordance with defined referral criteria and personal discussion.

The post holder will act in an advisory and educational capacity providing a service across the hospital, community and hospice setting to patients and families.

  • To work directly with patients and families using individual, group or family therapy techniques, according to defined referral criteria.
  • To support the development and provision of bereavement services
  • To provide knowledge and advice to staff in the Specialist team on the psychosocial and psychological support needs of patients and carers requiring Specialist Palliative Care support
  • To act as the patient’s advocate in bringing to the attention of appropriate practitioners the real or perceived unmet needs of the patient and/or their carers according to the referral criteria identified by the multi-professional team.
  • To promote collaborative working and a standard approach (where applicable) between LUHFT, the Hospice and community, social services and agencies in the voluntary sector. Liaising with those services to ensure that all families receive appropriate ongoing
  • To attend the Clinical Multi Disciplinary Team (MDT)meeting, theintegrated community, hospice and hospital MDT,meeting, the ambulatory MDT and the IMPaCT huddles and ensure that all other team members are conversant with all relevant information.
  • To operate within defined referral criteria as agreed by the IMPaCT service
