Join our team of competent and compassionate professionals as an Assessment Nurse in the One Care program at Behavioral Health Network!

The Assessment Nurse conducts the MDS (Multi Data Set) health assessment for members in the One Care program with histories of medical issues, behavioral health issues and/or substance use issues. Assessments are conducted in the members’ homes, through a virtual tele-health platform or telephonically.

Your experience, knowledge and great skills will support the One Care program with the following:

• Conduct initial and annual health assessments in way that promotes members’ health, safety, dignity and respect.
• Collaborate with members to develop person-centered care plans.
• Maintain a collaborative and professional relationship with the funding source, staff, members and stakeholders to support the care of members.
• Participate in routine nurse and team meetings.
• Complete all paperwork and documentation in accordance with the requirements of the funding source.

One Care Program Holds each individual at the core of what it does. Through the coordination and integration of care, One Care assists adults ages 21-64 with medical, behavioral health, or substance abuse concerns in navigating the health care system and improving their health outcomes.

What are we looking for in an Assessment Nurse?

• Current R.N. License by the Massachusetts Board of Registration.
• Previous experience providing services individuals with behavioral health and substance use issue is strongly preferred.
• Bilingual, Spanish and English Speaking is preferred.
• Must have familiarity with internet, email and electronic health records.
• Valid driver’s license required.
• Vehicle should be Registered and inspected in accordance w 540 CMR 4.00.
