Job Description POSITION TITLE: Gerontologist, Client Services Manager, Concierge Care Navigators HOURS: 40 hours per week FLSA CATEGORY: Exempt REPORTING TO: Program Director – Concierge Care Navigators The Council on Aging-Southern California is a private, nonprofit corporation, serving older adults and individuals with disabilities in Orange County since 1973. The Council’s mission is to promote the independence, health, and dignity of older adults through compassion, education, and advocacy. Concierge Care Navigators (CCN) is an affordable fee-for-service geriatric care management program designed to help family members navigate the complexities of a loved one’s health care needs. Our care management team, led by a Registered Nurse and Gerontologist, work together to provide older adults and their families with a customized care plan that promotes longevity and wellness. RESPONSIBILITIES: Customer relationship management including responding to client inquiries, maximizing client satisfaction, securing new clients, leading operations management, providing administrative oversight and reporting, conducting outreach presentations to introduce CCN program and benefits, attending networking events and collaborating with RN Supervisor and nursing team. REPORTING TO: Program Director, Concierge Care Navigators JOB FUNCTIONS (Including but not limited to): * Answer and manage incoming calls, e-mails, and website inquiries * Skilled at active listening to identify caller’s concerns/requirements * Educate callers on community resources and describe the services provided * Share knowledge of Orange County community resources * Utilize client management software to create and maintain client records * Perform follow up calls to providers and clients based on instructions from nursing team * Prepare service reports by collecting and analyzing client information * Collaborate with providers and internal staff * Schedule and participate in regular team meetings * Distribute customer satisfaction surveys and tabulate results * Develop and implement strategies to secure new clients and referrals in alignment with budget * Identify prospects, initiate communications, schedule meetings and presentations * Conduct outreach at community events, health fairs, networking events and small groups * Develop and maintain partnerships with organizations serving older adults * Forge relationships with community leaders, hospital staff (including physicians, discharge planners and case managers), wealth managers, fiduciaries, accountants, financial advisors, and others. * Assist in the content development of outreach, marketing and educational materials promoting CCN * Track outreach data and results to optimize efforts * Other duties as assigned by supervisor. May be required to flex schedule for weekend or evening meetings or presentations on a limited basis. QUALIFICATIONS: * Bachelor’s Degree in Gerontology, Human Services, Social Science, Business Management, or related field. Master’s Degree preferred. * Excellent written and verbal communication * Proactive, self-starter capable of working with minimal supervision * Expertise in public speaking to small and large groups * Well organized with the ability to multi-task * Proficient in the use of Microsoft Word, Outlook, and PowerPoint * Understanding and experience in Social Media platforms * Ability to work effectively with people in a variety of situations * Valid California driver’s license, proof of insurance and reliable transportation * Proof of eligibility to work in the United States * Successfully complete a Live Scan fingerprinting background check * Proof of COVID-19 vaccination or valid exemption WORKING CONDITIONS: The physical demands described here are representative of those that must be met by the employee to successfully perform the functions of the job. Standing, walking, and bending are required movements. The employee will occasionally need to lift approximately 20 lbs. and move furniture such as chairs and folding tables.
