Your Specific Responsibilities:
Supports agency quality improvement (QI) functions, including the strategic plan and divisional goals.
- Serves as a lead facilitating member of the agency Performance Management Council (PMC), acting as a liaison to the QI Council
- Responsible for the timely submission for the following foundational plans and annual progress reports: Performance Management and Quality Improvement Plan (PMQI), Workforce Development (WFD), Strategic Plan (SP), Community Health Assessment (CHA), Community Health Improvement Plan (CHIP), and Annual and Final Progress Reports (APR).
- Implements QI tools and techniques to support divisions and programs in their unique goals and objectives and other performance measures as needed.
- Develops and maintains performance monitoring dashboard for agency.
- Collects, analyzes, and maintains data and information related to agency performance measures.
- Provides technical assistance for PMC and agency divisions and programs.
- Continuously monitors agency performance, including strategic plan and CHD performance snapshot, identifies opportunities for improvement, and proposes and monitors corrective action.
- Ensures overall strategic and performance management efforts are aligned to and from agency and individual employee level.
- Participates in Regional PMQI Consortium meetings and additional training opportunities related to PHAB Reaccreditation
Supports agency public health accreditation planning and reporting activities.
- Oversees agency requirements to maintain Public Health Accreditation Board (PHAB) accreditation documentation and reports as it pertains to strategic planning, quality improvement, and performance management.
- Takes a lead role in documentation and monitoring public health accreditation standards and measures.
Supports agency Community Health Department to plan and conduct countywide program to improve health. The Community Health Department is comprised of the following programs: Healthiest Weight/Healthy Communities, Florida Healthy Babies, Reach Out and Read Program, Minority and Health Equity Programs, National Diabetes Prevention Program, Age Friendly Program, Community Health Promotion, and Opioids Prevention and Outreach.
- Community outreach and health promotion
- Countywide promotion and marketing of health campaigns and messaging
- Facilitate group presentations
- Development of partnerships to address chronic disease
- Research and utilizes data to assist in monitoring grant and program activities to aid in successful achievement of program objectives and goals

Qualifications:
Minimum –
- Willing to work before, during, and/or beyond normal work hours or days in the event of an emergency. This may include working in a special needs or Red Cross shelter, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural
- Have a current valid driver’s license or State ID. If out of state a Florida driver's license or State ID will be required within 30 days of hire
- Bachelor’s degree from an accredited college or university. An official transcript is needed to confirm your qualifications for this position
- Verifiable work experience in strategic planning, quality improvement, performance management experience and accreditation
- Verifiable work experience in community outreach and health promotion
- Verifiable work experience in facilitation of a group, coalition, or partnership
Preferred –
- Experience with Lean Six Sigma
- Experience in strategic planning, quality improvement, accreditation, and performance management
- Experience working in Public Health
- able travel, weekend, and evening hours
- Able to transport supplies, lift a minimum of 75 lbs., set up and breakdown of display, tents, tables and chairs at outreach events
- Access to personal reliable transportation
- Bilingual (English and Spanish)
