Family Care Connections in Glendale, AZ is seeking a full-time Habilitation writer. The role of a habilitation writer supports FCC coordinators with a variety of duties, including but not limited to; creating measurable goals and strategies for planning documents, inputting habilitation goals into our tracking system, requesting documents from support coordinators, assisting DSPs with executing habilitation goals, train & support new habilitation providers and maintaining records.
Required at minimum 1-year experience working as a habilitation provider, preferred 2 years of experience as a direct support provider. Being kind and friendly is important to FCC. Must be comfortable with working on a computer for most of your workday.
