An exciting opportunity has arisen for motivated, enthusiastic and compassionate individual to join us in working as part of the palliative care team on St Bede’s. The unit is a 10 bedded area which cares for patients with a range of life limiting conditions coming towards End of Life Care.

The successful candidate would be expected to work as part of the team to deliver high standards of patient care responsive to patient’s needs. There would be an expectation that you would be flexible and able to work to work a variety of shifts days, nights and weekends in order to accommodate the needs of the service.

To undertake and record urinalysis results and report accurately, to undertake and record blood sugar results and report accurately ·

Be able to operate machinery such as patient hoists.

Be able to stand for prolonged periods of time and mobilise for periods up to 12hours.

Gateshead Health, based in the North East of England provide a range of hospital and community health services from our leading facilities including the QE Hospital, Blaydon urgent care centre and Bensham. Since 1 October 2016 a new organisation called the Gateshead Care Partnership has been providing NHS community services across the borough by bringing together teams from the NHS, Community Based Care and Gateshead Council.

This innovative new partnership enables the whole system to work together more effectively so that experts from doctors, nurses, GPs and social workers who work together on better, more responsive care. By coming together in this way we bring everyone into a single organisation with patient care at its heart.

Dimensions

• To assist and support professionals from the multi-disciplinary team with agreed clinical activities. · You will be expected to have core skills training including, maintaining patient confidentiality, baseline observations, blood sugar estimations, urinalysis, resuscitation, infection control, safeguarding, and information governance. · Basic knowledge of electronic equipment and your role regarding basic maintenance and cleaning. · Keeping the nurse in charge informed of the patient’s condition. · To assist with hygiene of patients e.g. bathing patients in bed/bathrooms, toilet needs, hair care, oral care, nails helping dress/undress and promotes independence. · Performing Last Offices and preparation of the deceased person. · Assist with preparing patients meals and beverage’s, serving and feeding patients when they are unable to feed themselves. · Assist with admissions and discharges of patients eg. Listing patients belongings, routine admission swabs, ensuring wrist band is insitu. Strip and make beds and prepare immediate area and check and replenish equipment and stock. · Establish and maintain good relationships with patients, their relatives and colleagues. · Supporting and escorting patients when being transferred from one care setting to another · Contribute to the protection of patients from abuse and maintain patient safety. · Promote equality for all patients from abuse and maintain patient safety · Demonstrate effective communication methods within the work environment. · Comply with trust and local guidelines in load handling techniques, including patient mobilization. Demonstrate correct use of moving and handling aids and equipment. · Have an awareness of health and Safety legislation and support the maintenance of a safe environment. · To be aware of fire prevention, position of fire equipment and procedures to be adopted in case of fire or other emergencies · To attend ward meetings and in-service mandatory training events as required. · Understand the digital systems within the trust. · Clean and tidy clinical area and storage cupboards and demonstrate an awareness of infection control measures. · Undertake clerical activities at the direction of the nurse in charge, particularly in the absence of ward clerk · . To be involving in on-going training and development opportunities and contribute to your own personal development.

Communicates information to staff, patients, relatives and carers.

• Communicates information in an empathetic and reassuring way where there may

be barriers to communication/understanding i.e. speech/language/hearing

difficulties.

• Communicates with patients, clients, relatives and staff whose first language may

not be English.

• Is able to communicate effectively using written and oral skills.
• Establish and maintain effective relationships with patients, relatives and

colleagues.

• Provide and receive routine information requiring tact or persuasive skills; barriers

to understanding.

• Exchanges factual information with patients using persuasion, reassurance, tact, empathy, may overcome barriers to understanding e.g. patient has physical or mental disabilities.
• Acts as an ambassador/role model for St Bedes Palliative Care Unit and the Organization.

Able to prioritise the delivery of care to patients.

• Able to assist in the delivery of care of patients attending the outpatient clinic in

accordance with trust and departmental protocols.
