The heart failure nurse is a registered professional nurse who works in collaboration with physicians and multidisciplinary teams in the development, implementation and maintenance of all aspects of excellent quality care for the heart failure patient population.

The Heart Failure Nurse has a strong knowledge base in the care of cardiovascular patients, specifically heart failure patients, and its related processes. This position will organize and coordinate the care of Baptist Health Paducah patients for the health-systems clinicians, and in doing so, will have frequent interaction with the patient and others involved in the patient’s care. Much of the work is done at the hospital based clink, but an important aspect is this position’s ability as a work in the heart failure clinic. Important skill sets to possess are organizational, people-oriented, and self-starter skills. Since this position represents, on a daily basis, our health-system, a strong sense of professionalism is vital.

The following is a summary of the major functions of this individual's job. He or she may perform other duties, both major and minor, which are not mentioned below, and specific functions may change from time to time.

1. Contribute and lead hospital process, organization and coordination related to heart failure patients.

2. Work in a multi-disciplinary team during clinic to serve heart failure patients.

3. Accurately documents verbal and written communication involving patient care.

4. Assist in the identification of patients for participation in research trials; other research duties as needed.

QUALIFICATIONS AND ACCOUNTABILITY –

  • Current and active licensure by the Kentucky State Board of Nursing is required, BSN preferred.
  • 2 years of cardiology experience preferred. Knowledge of cardiovascular assessment, pathophysiology of cardiovascular disease, current medical and surgical treatments, and emergency procedures preferred.
