Early Intervention Service Coordinator
Location: Lebanon Ohio

Job Description:

  • Early Intervention serves children aged 0-3 that need services to address developmental delays or concerns. The Service Coordinator will provide support and consultation to referred families in a home-based setting. They take primary responsibility for the development and implementation of the Individual Family Service Plan (IFSP), which is developed with the family. They also coordinate all necessary services/treatment, referrals, assessments, and timelines. Service Coordinators work in collaboration with other community providers and provide referrals for other services the family may need. Responsibilities are assigned on a flexible day and evening schedule as determined by the needs of families served and the needs of the agency.

Qualifications:

  • Associates degree, Bachelor’s Degree preferred, in child and family studies; child development; child life; education inclusive of early childhood, pre-kindergarten, elementary education, deaf or hearing impaired, blind or vision impaired, special education or family life education; hearing and speech sciences or speech-language pathology; human development or ecology; human social services; nursing; occupational therapy; medicine; physician assistant; physical therapy; psychology; counseling; or social work.
  • Work experience with young children and their families; familiarity with early intervention services for infants and toddlers.
  • Knowledge of community agencies/resources, school district services and a demonstrated ability to work collaboratively with local agencies.
  • History of good job attendance
  • Ability to work flexible schedule to meet the needs of families.
  • Great communication skills; verbal and written
  • Excellent organizational skills
  • Ability to work independently and as a team member
  • Valid Ohio driver’s license and access to insured motor vehicle.
  • Ability to lift moderate amounts of weight and maneuver stairs unassisted.
  • Pass criminal background check.

Essential Functions:

  • Coordinates evaluations, assessments, and screenings of children who have or are suspected to have developmental delays.
  • Conducts family-directed assessments and collects information regarding the family’s concerns, priorities, and needs for early intervention services.
  • Assist families in finding and connecting with early intervention services
  • Provides ongoing visits in the family’s home.
  • Completes initial Individual Family Service Plans (IFSP) and conducts periodic reviews with family and early intervention service providers.
  • Promote positive parent child interaction, and provide families with appropriate child development information.
  • Plans and organizes own work to meet priorities and program timelines.
  • Complies with state and federal regulations pertaining to the Service Coordinator position (Ohio Revised Code 3701-8)
  • Maintains paper and electronic files with required documentation on all children served and follows confidentiality policies.
  • Provides services which are culturally sensitive and respectful to families.
  • Complete required trainings to receive Service Coordinator Credential and be responsible to renew the Credential every 2 years; continually improve professional competence.
  • Participate in child find activities and collaborate with other agencies.
  • Explores future planning with family, including transition to preschool services option, initiated at age 27-30 months; facilitate communication with school district and/or other agencies as needed or required.
  • Perform other duties consistent with the position assigned.

Other Duties and Responsibilities:

  • Serves as a role model for educators and students in how to conduct themselves as citizens and as responsible, intelligent human beings.
  • Helps instill in students and staff the belief in and practice of ethical principles and democratic values.
  • Shall perform such other duties as the Governing Board may assign/determine.
  • The employee shall remain free of any alcohol or nonprescribed controlled substance abuse in the workplace throughout his/her employment in the County/District.
  • Observes Risk Reduction Safety Rules and Regulations of the Warren County ESC and School Districts, State, Federal OSHA laws and will be evaluated to the extent of compliance and implementation as an employee of such rules and regulations for safety in the workplace while visiting such sites in line of work duties.
