Job description

We are seeking professionals with experience in processing new hospice referrals. Determines eligibility and benefits for new referrals or readmissions. Obtains authorizations and secures financial obligations for new referrals or readmissions. Coordinates receipt of information with clinical departments. Performs a variety of tasks, including contacting patients prior to start of care to confirm information, notification of financial obligation, and arrange any request for payments.

JOB REQUIREMENTS

• Initiates intake process gathers relevant data from referral sources and completes intake paperwork in the EHR software.
• Verifies eligibility to benefits.
• Obtains authorization for services and or pre-determination of benefits.
• Notifies clinical departments of new referral and coordinates information between referral source and clinicians.
• Schedule staff for admission.
• Schedule IDG staff after the admission
• At least 1 year of experience intake, hospice, or home health care.
• Disciplined self-starter
• Ability to multi- task
• Previous work experience in medical environment or medical education.
• Communication with other members of the patients’ multi-disciplinary team
• Strong computer skills
