$2000.00 SIGN ON BONUS Fast growing Rehabilitation Company is looking for a part time and per diem Physical Therapist to work in the West Haven, Branford, East Haven, Hamden, Milford and North Haven areas .Must be able to stand, bend, stoop, kneel and lift a minimal of 50lbs. Also, must have your own transportation.

POSITION SUMMARY:

Physical Therapist provides services to patients as directed by the plan of treatment in the patient's homes. Supervises Physical Therapy Assistants and maintains responsibility for the implementation and follow through of PT procedures.

POSITION QUALIFICATIONS:

Licensed as a Physical Therapist by the State of Connecticut, has graduated from a PT curriculum approved by the American Physical Therapy Association or the Council of Medical Education and Hospitals of the AMA and a minimum of one year experience as a Physical Therapist.

JOB DESCRIPTION:

- Demonstrates organizational skills and efficiency in performing duties

- Assists the physician in evaluating patient's level of function, establishing treatment plan and goals

- Performs initial evaluations within 48-72 hours of referral and re-evaluations as appropriate coordinating plan of care with the physician as needed

- Prepares clinical and progress notes and required summaries as appropriate

- Advises and consults with patient's family and other health care staff
