Carilion Clinic is a drug-free workplace.Carilion Clinic Home Care Services is seeking a full time Home Health RN Case Manager position for the New River Valley office located in Radford, VA office. This position will provide care primarily in the Giles region providing care in a variety of home settings. This position will support Carilion’s hallmarks of service excellence and quality for all job responsibilities. As part of the health care team, assumes primary accountability for managing nursing care and provision of individualized care for patients of all ages. Utilizes delegation and critical thinking skills to provide quality service through a team oriented, collaborative approach to patient care.

MINIMUM QUALIFICATIONS

Education: Graduate of school of professional nursing.

Experience: For Home Health, one year of experience is required with home health experience preferred.

Licensure, certification, and/or registration: Currently licensed as Registered Nurse in the Commonwealth of Virginia or new graduate of approved program eligible to sit for the Virginia licensure exam. For Home Health, must have valid Virginia Drivers License and pass a Criminal Record Check as per Virginia state law. Must obtain /maintain AHA BLS-HCP.

Other Minimum Qualifications: Must successfully complete orientation and competency requirements. Keyboarding skills required for automated computer systems, including electronic medical records. Must be able to meet essential job functions. Must have dependable transportation and may be required to travel in adverse weather conditions. Must have excellent documentation and organizational skills.
