Veteran Services Coordinator Monday-Friday with occasional weekend events. Minimal travel to off-site locations.

Job Summary~ Empathetic approach to assisting Veterans to become better educated, employed, and permanently housed. Case Manager will assist clients in meeting critical needs, such as: food, clothing, budgeting, and such that may affect their housing. Great emphasis will be given on developing marketable job skills that will financially support the consumer. Caseload will be between 20-25 single adults.

Essential Functions:

• Provide outreach to all partner agencies for referrals
• Appropriate intake and assessment of the consumer’s situation
• Provide case management, including, but not limited to:
• Self-sufficiency
• Budget management (including debt reduction and savings plan)
• Securing additional income, via employment or entitlement benefits
• Substance abuse and/or mental health care
• Referral and follow up with other appropriate services, including all VA benefits
• Other activities pertinent to maintaining permanent housing
• Outreach with potential landlords for partnerships.
• Assist in securing appropriate housing for consumers
• Maintain appropriate documentation of all client contact
• Maintain follow-up contact with post-residents
• Make off-site visits, as necessary
• Advocate on behalf of clients best interest when necessary
• Conduct outreach with potential partner organizations in assigned service area.
• Attend various Veteran focused meetings to network and outreach.
• Participate in Veteran events such as Stand Down.
• Maintain documentation /statistical information to generate weekly reports via HMIS
• Meet weekly with case management staff

Job Type: Full-time

Pay: $18.00 per hour
