Good Shepherd Hospice is a member of Catholic Health Services of Long Island and an integral part of the health care ministry of the Diocese of Rockville Centre. Our organization promotes the physical, emotional and spiritual well-being both of the person completing life's journey and their family members, providing comprehensive and compassionate end-of-life care of the highest quality. The
Hospice Liaison is responsible for assessing, planning and developing referrals. This position will establish and maintain professional relationships with key physician groups and referral sources, as well as act as a liaison to referral sources on behalf of Catholic Home Care and Good Shepherd Hospice and identify potential opportunities for market growth and program expansion.
The ideal candidate will possess excellent interpersonal, marketing and customer service skills, and act in a professional and supportive manner at all times.
Minimum Requirements:

Current and valid NY State license to practice as a Registered Nurse

Minimum of one year acute care hospital experience required

Proficient with Microsoft products

Two years hospice experience preferred

Bachelor's degree preferred
