Sentara is currently hiring an Intake Coordinator (RN or LPN) This is a Flexi position with rotating shift hours!

Position is located in Chesapeake, VA.

Job Requirements: RN-Associate's Degree ORRN-Bachelor's Level Degree or LPN

Degree Required: Nursing - 1 year Preferred: Home Health

Responsible for the coordination of care for patients admitted to home health, hospice or infusion in accordance with professional licensure, established procedures and regulatory requirements. Coordination of care includes coordination and direction of the interdisciplinary team, assessment of patient and physician orders to include other services that would be appropriate for the care required for the patient. Must be able to exercise independent judgment consistent with professional licensure in order to formulate an individualized plan of care for the patient in coordination with referring physician.
