Position Summary

The Wellness Center Intake Specialist is responsible for serving as the initial point of contact for all inquiries regarding treatment services, completing phone assessments, ensuring all necessary documents are understood and signed by clients, providing a thorough orientation during intake appointment, building rapport with community partners, and performing outreach efforts to educate the community on services provided by the program.

Essential Functions

  • Ensure all intake calls and/or emails are answered professionally. Responsible for the timely answering the Drug Medi-Cal phone line during business hours as well as scheduling all intake appointments for assessment.
  • Update the SBAT daily and keep daily census. Update intake spreadsheets daily.
  • Manage intake calendar and complete the intake process including completing all documentation requirements. Perform the initial orientation on the same day as arrival. Must meet timeliness standards of Los Angeles County Substance Abuse Prevention and Control Standards.
  • Perform timely data entry and reporting in HMIS, SBAT and SASH systems.
  • Maintain compliance with all licensing, contractual, HIPAA and programmatic requirements
  • Make and track all TB tests and Physical Exams required by DMC/SAPC, the VA and TSA.
  • Assist case managers with discharging patients and making sure all follow up appointments are made and communicated to the patient in a timely manner.
  • Responsible for all intake and discharge documentation and its accuracy.
  • Create and maintain data related to the census, daily client roster and contractual assignments
  • Review and monitor contract sign-in sheets daily and assist with necessary billing documentation and requirements.
  • Conduct daily phone screenings for program eligibility, schedule, and complete daily intakes for potential participants for all programs.
  • Ensure that all incoming participants are run through registered sex offender registries prior to entry into program.
  • Ensure that all entering program participants are enrolled into HMIS system with 48hrs of entering program. Assist in assigning and transferring files to case managers within 48hrs of participant’s enrollment.
  • Ensure and assist in creating badges for all participants and staff within one (1) business day of participant’s enrollment or request of replacement card.
  • Responsible for the accuracy and data integrity of participant data entered on and/or uploaded onto database and in the paper file.
  • Maintain communication with the Program Manager regarding intake schedule
  • Coordinate bed assignments and changes. Ensure basic essentials are provided at initial intake meeting such as basic hygiene, linen, and clothing.
  • Assemble information packets, including the DMC patient handbook, for all new patients
  • Enforce house rules and report notable incidents
  • Network with collaborative agencies and community partners for referrals/resources. Provide referrals and direct inquiries through appropriate channels
  • Attend monthly collaborative meetings and outreach events
  • Responsible for accurate data entry and daily compilation of occupancy statistics. Prepare accurate month end reports for contract guidelines and other reports as needed
  • Attend In-Service Training classes as scheduled
  • Drive company vehicles & transport clients to appointments, as needed
  • Must be able to work evenings, weekends, holidays, and temporarily beyond normal working hours and/or in other positions/departments, when necessary.
  • Establish and/or maintain positive working relationships with all other Bell Shelter employees
  • Perform all other duties as assigned by Wellness Center Program Manager, Clinical Director, or Facility Director

Working Conditions

Must be able to sit, walk, stand, bend, squat, climb, kneel and twist on an intermittent or sometimes continuous basis. Must be able to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead. Must be able to lift up to 25 lbs.

Due to licensing requirements, must be fully vaccinated and have booster shot or accept booster shot within 15 days of eligibility. This position requires employee to be vaccinated from Covid-19

Minimum Qualifications

  • Bachelor’s Degree is preferred along with one year experience in medical and/or psychiatric models a plus.
  • Certification/registration as an Addiction Treatment Counselor by state approved organization required.
  • Must have current TB test.
  • Must be CPR and First Aid Certified.
  • Knowledge of HMIS preferred
  • Class B Driver License preferred
  • Clean MVR and ability to drive & transport clients
  • Experience with co-occurring disorders a plus.

Skills, Knowledge & Abilities

  • Ability to carry out assignments independently and must possess strong time management skills
  • Strong verbal and written communication skills.
  • Working knowledge of addiction & 12-step programs
  • Computer literate in Microsoft Office Applications and ability to type a minimum of 45 WPM

Job Type: Full-time

Pay: $19.00 - $25.00 per hour
