What will you be doing:

The Intake Coordinator coordinates activities of order intake, evaluation of admission criteria, referral/authorization of cases, and transition of care for the Home Health Agency; staffs initial nurse, contacts patient/family member, and tracking of referrals. The intake coordinator acts as liaison to case management ensuring patient care.

What will you come with:

• Minimum of some college coursework
• Minimum of one (1) year of nursing experience, in home healthcare preferred
• Active Practical Nurse (LPN) license in the State of Florida.

Specialized knowledge

• Knowledge of the Home Health Services
• Knowledge of referral/authorization process
• Knowledge of computer clinical programs such EHR/EMR system(s)

Skills & Abilities

• Great communication skills, both written and spoken.
• Bi-lingual preferred
• Strong organization skills and be detail oriented
• Ability to maintain confidentiality of sensitive information.
