The
Licensed Practical Nurse (LPN)
,
Ambulatory Care and LPN, Advanced, will work
Full Time, Days and will provide direct and indirect patient care under the direction of the Registered Professional Nurse, Nurse Practitioner, Physician's Assistant, or Physician. The Licensed Practical Nurse functions as part of the interdisciplinary health care team. The LPN, Advanced serves as a resource for other team members and is active in the orientation of new staff.
The Licensed Practical Nurse collects and records relevant patient health data accurately, with no more than 3-5 exceptions, as measured by random supervisory chart review and/or direct supervisory observation and co-worker feedback.- The Licensed Practical Nurse evaluates health data to identify patient problems, with no more than 2-3 exceptions, as measured by random supervisory chart review and/or direct supervisory observation.
- Implements and documents specified interventions with no more than 3-5 exceptions, as measured by random supervisory chart review and/or direct supervisory observation.
- The Licensed Practical Nurse identifies practice problems and consistently seeks solutions through appropriate communication channels.
- Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity as measured by no significant, substantiated patient or peer complaints received by the manager/supervisor.
- The Licensed Practical Nurse demonstrates operational problem-solving ability through written exemplar, patient/family feedback, co-worker report or direct managerial observation.

This mandate requires COVID-19 vaccinations for personnel working for or on behalf of our hospitals and nursing homes. Education:
- Graduation from a Practical Nursing program.
License/Certification:
- NYS Licensure as a Practical Nurse.
- Current NYS Licensure as a Practical Nurse is required.
- Basic Life Support (BLS) is preferred.
