Total Care Group is a person-centered care organization that excels in providing care and supervision for adults with varying intellectual and developmental abilities, with an emphasis on the behavioral health. We have 18 residential homes throughout Southeastern Wisconsin, each with 2-4 residents that are ambulatory, active in the community, and uniquely engaging to learn and grow with.

Title: Licensed Practical Nurse (LPN)

Shifts: primarily 1st shift office hours, with ability to join our Management On Call calendar two days per week

What You Will Do:

• Build strong, professional relationships with our residents.
• Observe and report changes in client condition or any other transitions to the physician, family, and case manager in a timely manner.
• Generate and review Individual Service Plans (ISPs) and speak on client conditions and treatments in conjunction with our Director of Operations.
• Encourage Resident Assistant staff in proper charting practices and procedures, and provide additional guidance as needed.
• Maintain open communication with our Management Team regarding staff performance problems and successes.
• Demonstrate patience, tolerance, understanding, and a sense of humor.

What You Need:

• Possess a valid, active LPN license in the state of Wisconsin, and maintain current licensure.
• 1-3 years’ experience in the healthcare field is preferred, but not required.
• Possess a valid state of Wisconsin Driver's License.
• Ability to pass a background check.
• Ability to stand, bend, lift, and possess full ambulation.
