Requirements:
· B.A. degree in Social Work or Behavioral Sciences
· Active social worker’s license in Minnesota
· Be able to pass a DHS background check
· Be a strong collaborator and effective communicator, be organized, kind, and courteous to residents, families, staff, and vendor partners
Responsibilities:
· Ensure confidentiality, documentation, and follow through consistently and as needed
· Pre-Admission:
§ Collect necessary information on inquiries
§ Share general information about the facility
§ Tour families and prospective residents around the facility
§ Pre-screen with the Director and Assistant Director of Nursing to assure that a resident’s needs can be met by the facility
· Admissions:
§ Orient new residents and family to the facility
§ Review all admission forms with the resident, and/or family, and answer any questions
· Social History and Assessment:
§ Complete a social history for each resident within 10-day of admission
§ Update as necessary
§ Complete comprehensive assessment at time of admission and annually; review at least quarterly
· Care Plans:
§ Assist with developing care plan for each resident; review as needed and at least quarterly
§ MDS
§ Attend Multi-Disciplinary Team meetings
§ Coordinate care conferences with residents, family, and staff
· Grievance Official
§ Respond to grievances per regulations
§ Maintain records and a grievance log
§ Facilitate resolution
§ Communicate, discuss, and collaborate with Administrator, Leadership/Management team, and any other key players on grievances, issues, or concerns.
· Document progress notes at least quarterly
· Discharge Planning
§ Discharge coordination
§ Collaborate with resident, family, nursing, medical provider/physician, and community resources to maintain continuity of care upon discharge
§ Manage discharge summaries
· Community Resources
§ Maintain file of available resources and share with others
§ Make referrals as needed
· IDT
§ Attend and participate in interdisciplinary team meetings
§ Participate in quality assurance
· Service for Families
§ Assist and refer families to appropriate sources for financial arrangements
§ Investigate complaints and answer inquiries
· Additional Services
§ Assist with issues of residents, families, or service providers
§ Maintain and cultivate relationships with residents and families
§ Assist residents in obtaining necessary equipment and personal items (i.e. clothing, etc.)
§ Assist residents and/or families with completing forms as needed
§ Connect with other departments on room changes or resident placement
§ Assist resident in adjusting to and setting up room
§ Communicate notice prior to a room change to residents, families, and staff as needed
§ Participate in committees, as appointed, or sought out, to maintain quality of care within the facility
· Community Involvement
§ Represent the facility at professional organizations when appropriate
§ Complete ongoing education to update knowledge by attending seminars, workshops, and courses
· Social Work Department and Facility
§ Policies and Procedures: Update processes and manuals as needed
§ Safety: Practice safety precautions in accordance with the home’s safety procedures and attend required in-services
o Practice and promote Infection Prevention and Infection Control
§ Promote and market New Brighton Care Center
